In case of emergency, I understand Our Lady, Queen of Apostles Faith Formation will make every effort to contact me or the other designated emergency contact. However, if they cannot reach me or the designated person, I give my permission to call for emergency medical assistance. I release Our Lady, Queen of Apostles Faith Formation Team, staff, and parish volunteers from all liability of any kind which may arise from such an emergency.

General Permission & Release Form
I give permission for my child(ren) to participate in Our Lady, Queen of Apostles Faith Formation programming and release from all liability and indemnity the volunteers and employees of the Bishop of Columbus individually and as a trustee for the Diocese of Columbus, Our Lady, Queen of Apostles, and all parishes/schools and their representatives from any and all liability, claims, judgements, cost, and expenses, including attorney's fees, arising out of injury or illness incurred by my child while participating in or traveling to or from the activity and further agree not to bring or prosecute or allow to be brought or prosecuted (including but not limited to prosecution through subrogation) in my name, my child's name, or on behalf of my child, any claims, lawsuits, or actions against Our Lady, Queen of Apostles, the Diocese of Columbus, volunteers, and employees. I further understand that my child(ren)'s participation in this activity is purely voluntary and that my child, and I, on behalf of my child, agree to my child(ren)'s participation in Faith Formation despite potential risks.

During Faith Formation pictures may be taken for the parish website, newsletters, and social media pages. The names of your child(ren) will not be published with pictures.
